Connected Community of Freeborn County
St. John’s Lutheran Community in Albert Lea received a $600,000 grant from the LeadingAge™ Minnesota Foundation to:

Fund a “Connected Communities” initiative to support rural seniors in Freeborn County to stay in their own homes.

Introduce the “Senior Living Without Walls” model to help seniors stay independent and age in place.

Partner with SMART Transit, Albert Lea Fire Rescue, and Mayo Clinic Health System.

Integrate technology such as remote monitoring will improve healthcare access and reduce social isolation.
